Registered Nurse – Home Health

You will work closely with patients, families, and an interdisciplinary care team to deliver high‑quality, personalized nursing support.
What You’ll Do
Deliver hands‑on nursing care in patients’ homes, including assessments, wound care, medication administration, and monitoring of chronic conditions
Develop and update individualized care plans based on patient needs, goals, and clinical findings
Educate patients and families on health management, medication use, and safety practices
Communicate regularly with physicians, therapists, and other care professionals to coordinate patient care
Document all visits, observations, and interventions clearly and accurately
Respond to changes in patient conditions and adjust care appropriately
Support patients and their families with empathy, respect, and clear communication
Required Qualifications
Current Registered Nurse (RN) license
Clinical experience in home health, medical‑surgical, geriatric care, or a related area
Strong assessment, critical‑thinking, and problem‑solving skills
Ability to work independently and manage a flexible field‑based schedule
Valid driver’s license and reliable transportation for home visits
Nice to Have
Experience with electronic health record (EHR) systems
Specialized training in wound care, chronic disease management, or palliative care
Bilingual communication skills
